30+ Years Practice

Dr. Karem completed his undergraduate BA in Chemistry at Eastern Kentucky University and graduated from the University of Louisville School of Medicine in 1991. He completed an Internal Medicine and Pediatric residency in 1995 at the University of Louisville, then began private practice in Oldham County that same year — a practice he remained with for his entire career.

His philosophy has always centered on compassionate, individualized care. He believes a holistic approach — one that integrates optimal nutrition, consistent physical activity, and thoughtful medical support — provides the best foundation for long-term health. With the concierge model, he looks forward to offering the depth and time each patient deserves.

Metabolic Health

Dr. Neff trained at the University of Louisville for his undergraduate education, medical training, and residency, where he later served as Chief Resident. That experience shaped not only his clinical foundation, but also his leadership philosophy — collaborative, thoughtful, and committed to excellence.

After years in traditional corporate medicine, Dr. Neff watched firsthand as the system's structure made it nearly impossible to practice the kind of medicine he trained for. Fifteen-minute visits, insurance-driven priorities, and an ever-growing patient panel left little room for prevention, depth, or the kind of physician-patient relationship that actually changes health outcomes. He left corporate medicine to build something different — a practice where he controls the variables that matter most: time, access, and trust.

By limiting his panel to a small number of patients in the Louisville and Anchorage, KY area, Dr. Neff offers longer visits, direct communication, proactive health planning, and individualized strategies that address the root drivers of disease — not just the symptoms. His goal is simple: thoughtful, accessible, relationship-centered care designed to help you thrive.

Registered Nurse

With a strong foundation in healthcare, she began her medical career in 2009 as a medical assistant, gaining valuable hands-on experience and a deep passion for patient care. Over the past 17 years, she has worked alongside Dr. Karem in both private practice and within the Baptist healthcare system, building lasting relationships and a reputation for dedication and compassion.

In 2023, she made the decision to further advance her career by pursuing a nursing degree. Her hard work and commitment paid off when she graduated in December 2025 from Jefferson Community and Technical College with her Registered Nurse (RN) degree.

Outside of her professional life, she has been happily married to her husband, JP, for 18 years and is a proud mother of two daughters. She enjoys spending her weekends relaxing at the lake with her family, taking long walks outdoors, and treating herself to a good pedicure.
