Fees & Insurance

Do you take insurance?

Yes. We will be credentialed with most major insurance plans, including Medicare, and your insurance is billed for covered medical services you receive in our office. Your annual exam fee is separate — it covers your comprehensive annual exam plus your concierge benefits, like extended visits, 24/7 direct access, and personalized attention, and is not billed to insurance.

Can I use my HSA or FSA?

Depending on your individual plan, certain medical services provided as part of your care may be eligible for reimbursement through a Health Savings Account (HSA) or Flexible Spending Account (FSA). We are happy to provide documentation for medical services upon request. We recommend confirming coverage details with your plan administrator.

Can I cancel anytime?

Yes. We want you to feel confident joining our practice. Your terms and cancellation details will be outlined in your patient agreement. Please reach out to our office for specific details.

What do I need for my first visit?

Bring a photo ID and a list of your current medications and health history. We'll send any new-patient forms ahead of time through your patient portal so your first visit is unhurried.

How do I view my medical records?

You can access your records, lab results, and visit notes securely through your patient portal. Reach out anytime and we'll help you get set up.

Care & Access

What happens if I need a specialist?

We provide comprehensive care coordination. If you need a specialist, we will assist with referrals, communicate directly with the specialist team, and help you navigate the process. Your insurance can still be used for specialist visits and any additional services outside our practice.

How quickly can I get an appointment?

As a patient, you have access to same or next-day appointments for urgent needs. We intentionally keep our patient panel small so that you never have to wait weeks to be seen.

How do I reach my physician?

Patients have direct access to their physician via phone, text, or email for medical concerns and follow-up questions. You will no longer need to navigate a call center or wait on hold to reach your doctor.

Do you offer telemedicine?

Yes. Telemedicine visits are available for appropriate concerns, giving you flexible access to care from wherever you are.

Services & Treatments

Do you prescribe GLP-1 medications?

Yes. As part of our comprehensive weight management program, we evaluate candidates for GLP-1 receptor agonist therapy on an individualized basis. Treatment is guided by your medical history, health goals, and clinical appropriateness, with ongoing monitoring and support throughout.

Do you offer hormone therapy?

Yes. We provide evidence-based hormone optimization for both men and women, including testosterone therapy, perimenopause and menopause management, and thyroid optimization. All treatment is individualized, carefully monitored, and focused on safe, sustainable outcomes — for those who qualify for therapy.

What is included in the annual comprehensive exam?

Your annual comprehensive evaluation includes a thorough review of your medical history, preventive care, an EKG, pulmonary function testing, vision and hearing screening, cardiovascular risk assessment, and advanced lab interpretation. It is designed to give you a complete picture of your health, identify any areas to address proactively, and set a personalized health plan for the year ahead.

Are labs and imaging included in the annual exam fee?

Laboratory testing and imaging are not included in the annual exam fee, but we work to provide access to these services at transparent and often significantly reduced costs. We partner with providers to offer wholesale pricing wherever possible.

Getting Started

How do I become a patient?

The first step is a free discovery consultation. This gives us the opportunity to learn about your health goals, answer your questions, and determine if our practice is the right fit for you. From there, you can enroll as a patient and schedule your comprehensive intake visit.

How many patients do you accept?

We intentionally limit our panel to approximately 300 patients. This allows us to provide the level of attention, access, and personalized care that defines our practice. Availability is limited.

Do you see children and adults?

Yes. Our practice is uniquely positioned to care for both adults and children. Dr. Neff and Dr. Karem are board certified in Internal Medicine and Pediatrics, allowing the whole family to receive care under one roof.
